Thank you for choosing The Center for total Eye Care, for your vision needs. First visits usually take approximately 60-90 minutes. Dilation of your eyes will probably be required for the examination. Since the effects of dye drops can take several hours to wear off, it's best that you plan not to drive yourself immediately after the appointment and arrange for someone else to drive if possible. You should bring sunglasses also, even if it is a cloudy day. If you do not have any sunglasses, we can provide a temporary pair.
Our staff would be happy to submit a claim on your behalf for eye care services. Be sure to bring your insurance information along when you visit.
For your first visit an informational packet will be mailed to you. Please complete the enclosed Patient Registration Form and the Medical History Questionnaire. Please bring the completed forms with you to your appointment. Also bring a list of the medications and eye drops that you are currently using, along with glasses and/or contact lenses (no matter how old the prescription is). Include any over the counter and herbal medications in your list. Be sure to have your insurance card(s) and a referral form (if applicable), with you when you come to the office. If you do not have your insurance card or referral you may reschedule the appointment until such information is received or you may pay for the visit in full at the time of service.
